You may print, complete and bring the following forms with you to save time in our waiting room. This will allow us to maintain accurate contact information and medical records for you.


 

REMINDERS OF REQUIRED ITEMS FOR YOUR VISIT

• Insurance Card (Primary and Secondary) and Photo ID

If you have health insurance, we need a copy of your insurance card

(otherwise visit will be self-pay).

• Co-pay and/or Previous balance is collected at check-in

• Cosmetic procedure fees are due at time of visit

• Completed Patient Registration Package and Medical History

• Written Referral from your Primary Care Physician

(if required by your insurance plan)

• Parent or Legal Guardian must accompany patients under 16
