Participating Insurance Plans:
- United Health Care
- Blue Cross Blue Shield (participating, not preferred providers)
- Anthem
- Medicare
Please contact the office if you have questions about insurance.
We are in the participating network (PAR) of both Blue Cross Blue Shield and
Anthem. We will submit your claims for you and accept the allowable
amounts set by the insurance company for services. There is not a set
copay, first you must meet your out of network deductible and then your
insurance company will pick up a portion of the services (usually around 75%) ,
and you will be responsible for the remaining 25%. We are not part of
the preferred provider network (PPO). If you have any questions
regarding insurance please contact the office.
